선박에 승선하다 보면 여러 긴급한 상황에 직면하게 된다.

그 중에서도 황천으로 인한 화물 손상, 충돌, 좌초, 하역 작업 중 화물 손상, 인명 손상 등 여러가지 상황

발생시 필요한 statement of fact sample이다.

Port of Destination : Hong Kong

 

To whom it may concern

 

I, Capt. Gil-dong Hong, Master of M/V Star laden with 50,000 M/T of general cargo in containers, hereby state that vessel collided with M/V Dragon at Pos. 20-20.0N, 100-00.0E at 1030hrs of 21st Nov. while proceeding to Hong Kong P/S as follows :

 

While on sailing for Hong Kong to pick up pilot, own vessel was keeping on Co.273 Degree and, maneuvering full speed with stand by Engine, on the planned route, under my command with 2 Radars, 2 ECDISS, and steering gear which were working good.

 

We all member of bridge team, Master, 2/O, and Q/Master found one vessel, M/V Dragon on the cross situation in almost 10 clockwise direction about 3 miles away at 1020hrs same day. I was understanding that the M/V Dragon had to give way and should avoid crossing with the own vessel. So I, as the standing on vessel, maintained speed and course right up to the moment. As the M/V Dragon give-way vessel was closing to me little by little without any actions to avoid collision situation, I raised 5 blasts to warn not to get close with call on VHF ch.16 at 1025hrs same day but no answer for my call.

 

In spite of our actions, M/V Dragon, was closing the own vessel on collision situation, I, master, steered hard to STBD and then hard to port where I realized that the action of the give-way vessel, M/V Dragon, alone was not sufficient to avoid a collision and had to take avoidance action accordingly at 1029hrs same day. Soon after I did actions to avoid colliding, the M/V Dragon hit the hull on the port side of the stern quarter of the own vessel at 1030hrs same day.
